Member of the Executive, Ontario Pharmacy Evidence Network (OPEN) & Author, Pharmacy in the 21st Century

Lisa will share an inspiring vision for the near future of community pharmacy, which will see the "decoupling" of compounding/dispensing functions from  medication management and patient care services, as presented in Pharmacy in the 21st Century. This thought-provoking research document concisely outlines two key directives and 12 principles to guide the profession's transformation. 

Significant challenges, however, need to be overcome to achieve the substantial organizational change required within pharmacy and to improve external connections with patients, other healthcare providers and other healthcare organizations. Lisa will summarize reactions to the paper since its publication in January 2019, and where we can go from here.

Ashesh will speak to current initiatives to evolve pharmacy practice, while seeking to answer the following questions:

  • Where has pharmacy been most successful?
  • How are key challenges being addressed?
  • What are the main supports to help frontline pharmacists make the transition to patient-centred, team-based care?

We will also update you on some of the exciting projects funded by CFP's Innovation Fund, including:

  • The use of a communications tool to support the transition from hospital to community care;
  • The appointment-based model as a means to improve the delivery of clinical services
